Forms
Many
of our students are selected by the Federal Processor for "verification." If your Student Aid Report (SAR), which has
messages from the Federal Processor related to your FAFSA application,
indicates that you have been selected for VERIFICATION, contact us to be sure what documents you need to bring to our
office. All documents requested will need to be copies we can
keep. You will also need to complete and sign the appropriate verification form
below; be sure you and anyone else who needs to complete and sign do so before you come to our office.

Declaration of Major Form (if you have Adobe Acrobat Reader)- from Registrar's Site
Important for receipt of TAP and other NY State programs. See
details in the our information about TAP, APTS, and Part-time TAP on our programs page. Complete this form, have it approved and signed in your chosen academic
department, and submit it to the Registrar's Office - before you have
completed your 60th credit toward your undergraduate degree.
Direct Deposit
If you give authorization to the Bursar's
office, balances remaining for you in some types of aid can be
deposited directly into a bank account you specify, instead of being
mailed to you.
