Entrance Counseling and Promissory Note
Students awarded Perkins Loans must complete Perkins Loan entrance counseling and sign a Perkins Loan Promissory Note each academic year. The entrance counseling and the Promissory Note are to be completed on the web. *** The-2008-2009 Federal Perkins Loan Entrance Counseling and Promissory Note are now available. ***

Checks will not be produced, nor will monies be applied to the tuition bill or book vouchers issued for students who have not completed the entrance counseling or signed the Promissory Note. You will be asked to provide the following information :
Please read all provisions and the rights and responsibilities material. You are required to complete the quizzes and provide all the necessary information. Allow 1 hour to complete. You may print the Entrance Interview and Promissory Note. The school will be notified that you have completed the process. Keep your copies of the loan papers in a safe place for future reference. In order to receive information related to your Queens College Federal Perkins Loan we must have your current address on the Federal Perkins Loan billing system. Please notify either ECSI at (888)549-3274 or Ellen Rondot at (718)997-5111, in addition to all the other offices on campus, of any change of address.
Eligibility requirements for your loan include: you must be a matriculated student, registered for - and attending - at least 6 credits, not be in default of any prior student loan, be making satisfactory academic progress as defined for the federal Title IV aid programs, and have a minimum cumulative GPA of 2.0. Undergraduates must have completed 28 credits toward the degree. If you have been awarded a Perkins Loan, you may want to read carefully CUNY's booklet: Your Federal Perkins Loan: A Reference Guide of Your Rights,Responsibilities and Obligations. Please note that the Federal Perkins Loan and the William Ford Direct Loan are two separate and different loan programs. Information is available about all your student loans at http://www.nslds.ed.gov . You will need your financial aid PIN for the NSLDS site; if you don't already have a financial aid PIN, you can file for one at http://www.pin.ed.gov.
