Downloadable Grade Report Forms for Visiting Students

A  grade report will be sent free of charge to the home school if a request is filed with the Office of the Registrar by the last day of classes. Be advised that only one grade report will be sent. You may download a grade report form, fill it out, and return it to the Registrar's Office before the deadline. Grade reports are not sent to the home school unless officially requested by the student.
If your request is submitted after the last day of classes, you will need to order an official transcript. You may ask that an official transcript be sent to your home school at any time. Transcripts can be requested in person at the Office of the Registrar, Jefferson Hall, first floor; online; or by downloading the request form. Students are required to pay a $7 fee for transcripts going to non-CUNY schools. For additional information on how to order a transcript, please see our frequently asked questions site.

You may print the forms you need from this page, complete them, and submit them following the procedures described on each document. Most of the forms above can be completed before printing.

The period for students to drop courses for a full refund was scheduled to end Tuesday January 27.  However, since some students may have had difficulty changing their schedules due to the blizzard, we have extended the 100% refund period to Sunday  February 1st at 11:59 pm.  On Monday, February 2nd and Tuesday, February 3rd, courses may be dropped for a 75% refund, and  the 50% refund period will commence on Wednesday, February 4th, as normally scheduled.  If you need to drop or add courses, please take this additional time to make changes without any financial liability.
