1- Last day of Weekday Classes Summer Session 2L is August 07, 2008.


2- Visiting students should submit to the Registrar's Office a request for an official grade report for their home college by August 07, 2008. The form can be downloaded and mailed before the dateline. After the dateline a fee will be required.


3- Grade reports for the four summer sessions will be emailed via the student's QC Lotus Notes email account by August 25, 2008.


4- All duly registered Queens College students must have a valid Q.C. e-mail address. To sign up click here.


5- Accessing eSIMS using http://esims.cuny.edu was discontinued on July 15, 2008. You should use your CUNY Portal, Username and Password to access eSIMS. If you have not already done so, please go to http://portal.cuny.edu , to obtain a Portal Username and Password.

This guide will answer some of the most commonly asked questions about transcripts. It will provide you with important information on how to request transcripts.

1. How can I obtain a transcript?

    You can order a transcript three ways:

(a)

Order Online - The quickest and most convenient way to order a transcript!

(b)

Download and complete a transcript request form and mail it to the Office Registrar with the appropriate fee.

 

(c) In person by visiting in the Office of the Registrar, Jefferson Hall, 1st Floor.

2. Who provides the Online Transcript Service?

Queens College has partnered with Credentials Inc. to accept transcript orders via the Internet through a secure site. Transcripts may be ordered at your convenience, any time, 24 hours a day, seven days a week.

3. What do I need to request a transcript online?

To utilize the online service, you will need to provide your Social Security Number or Student ID number; telephone number; a valid major credit card (VISA, Master Card, American Express or Discover) in addition to other information requested by Credentials Inc. Please be aware that your transcript request will not be processed if there are any "stops" on your record (ie. Bursar, Library, Financial Aid etc.). There is an additional $2.00 fee to use this service.

5. Can I order transcripts online for academic departments within Queens College?

NO. You do not need to order transcripts online for academic departments, departments can access your record, if necessary. If a department is requesting your transcript because it is going to be forwarded to another institution, you will need to download a transcript request form, fill it out and mail it to the Office of the Registrar with the appropriate fee if the transcript is ultimately forwarded out CUNY.

6. What method of payment can I use?

7. What type of transcripts do you issue?

There are three types of transcripts issued by Queens College:

(a) STUDENT TRANSCRIPT * issued & mailed to students.
(b) OFFICIAL TRANSCRIPT * mailed directly to employers, schools or other institutions.
(c) OFFICIALLY SEALED TRANSCRIPT **

mailed to student, only when the transcript request indicates the name of the institution or employer, the city and the state where the transcript will be forwarded to by the student. Typically an officially sealed transcript is required when it must be included with an application packet, which will be forwarded by the student to the institution or employer.

Please note: transcripts to CUNY schools are mailed directly there via Inter-College mail. If the transcript is to be mailed to you, please provide documentation from the school to support your request.

* Official and Student transcripts may be ordered via the Internet through a secure site.

** Officially sealed transcripts - you need to download a request form, fill it out, print and mail to the Office of the Registrar.

8. What information do I have to include, if I am mailing in a written transcript request?

You need to include the following information:

Type of transcript being requested

Information needed
Student Transcript

The letter must include the student’s name, other name used if applicable, identification number or Social Security number, address, telephone number, date of birth, dates of attendance, and graduation date, if applicable.  In addition, you need to specify that you are requesting a student transcript and the letter must be signed by the student.

 

Official Transcript

The letter must include the student’s name, identification number, address, telephone number, date of birth, dates of attendance, and graduation date, if applicable.  In addition, the name of the institution or employer and the forwarding address.

 

Officially Sealed Transcript

The letter must include the student’s name, identification number, address, telephone number, date of birth, dates of attendance, and graduation date, if applicable.  In addition, name of the institution or employer, the city and state where the transcript will be forwarded by the student to the institution or .

Please note: transcripts to CUNY schools are mailed directly via Inter-College mail. If the transcript must be mailed to you, please provide documentation from the school to support your request.

9. What is the fee for a transcript?

The fee is $7.00, U.S. currency.

10.  Do I have to pay $7.00 for transcripts requested being sent to any branch of the City University of New York (CUNY)?

No.  Transcripts sent to CUNY schools are free of charge.

11.  Must I pay $7.00 for an undergraduate transcript and another $7.00 for a graduate transcript?

No.  If both requests are made at the same time, there is only one fee.

12.  If I have two degrees, may I request a transcript for only one?

Yes.  You may request a transcript of one or both degrees for a fee of $7.00.

13.  Is the University Application Processing Center (UAPC) a part of CUNY or do I have to pay for a transcript?

Yes . The UAPC is a part of CUNY and, therefore, the transcript is free of charge.

14.  May I leave my UAPC application with my request for a transcript, or do I have to mail it separately?

Either way is fine.  However, if you have a deadline, it is better to mail the application separately.  In addition, if you leave the application with the Office of the Registrar and are not currently attending other units of CUNY, you must include the UAPC fee of $70.ºº.

15. Is there any way that a student can obtain an official transcript in a sealed envelope?

Yes, by providing the name of the institution or employer, the city and the state where the transcript will be forwarded. If the transcript is going to a CUNY school; provide documentation or a pre-printed envelope from the CUNY School with this form.

16. Can transcripts be released without a student’s consent?

Under usual circumstances, no.  This is in compliance with Public Law 93-380, “The Family Rights and Privacy Act.”  However, if a subpoena is issued, the Office of the Registrar is required to comply.

17. * What are the peak times?

Peak times are the first two weeks of the semester, last two weeks of the semester, during pre-registrations, and during registration days.

18. How long does it take to process transcripts?

Normally, one to two business days after the request is received.  For students enrolled prior to Spring 1992, processing requires two to three business days. However, the request can only be honored at off-peak times.*

19. If I am in attendance this semester (current semester), can I have my grades included on my transcript?

If you want to ensure that your grades are posted on your transcript, you may request that the Transcript Unit hold your request until grades are posted. You must wait at least three to four weeks after the term has ended before the service is available.

20. If I am graduating at the end of the current semester, will my degree notation be posted on my transcript?

Yes.   We will honor that request.  The Office of the Registrar will hold your transcript request until the degree notation is posted. You can expect to receive the transcript a week after the conferral of degree date.

21. Can I pick up a transcript?

A transcript can be picked up two business days after the request is received. However, the request can only be honored at off-peak times.*

22. Is same day transcript service available?

If you were in attendance after the Spring 1992 semester, and you need to meet a deadline, we will process your request within two business days after the request is received.  However, the request can only be honored at off-peak times.*

23. If I want my current grades included on my transcript, can I order the transcript now ?

No. If you want your current grades included on your transcript, you need to download a transcript request form, check the box to hold for grades, fill out all the appropriate information, enclose payment and mail to the Registrar's Office. You must wait at least three to four weeks after the term has ended before the service is available.

24. If I am graduating at the end of my current semester, can I request my transcript now?

No. If you want your graduation notation included on your transcript, you need to download a transcript request form, check the box to hold for degree notation, fill out all the appropriate information, enclose payment and mail to the Registrar's Office. You can expect to receive the transcript a week after the conferral of degree date.

25. Will I be notified if there is a problem with processing my transcript?

Yes.  The Office of the Registrar will attempt to reach you by phone and/or mail if there is any difficulty in processing your transcript request.  All accompanying documents and a letter explaining the reason why your request was not granted will be returned by mail.

26. If there is a hold/stop on my record, will it prevent me from obtaining a transcript?

Yes.  All holds/stops will need to be cleared before the Office of the Registrar can honor your transcript request.

27. Can a transcript be e-mailed or faxed to other institutions?

NoTranscripts can not be e-mailed or faxed to students or institutions.

28. Can someone else request a transcript for me?

Yes, with student's authorization. The request must include a signed letter from the student along with a copy or the original picture ID with the student’s signature on it. The letter must include the student’s name, identification number, address, telephone number, date of birth, dates of attendance, and graduation date, if applicable.  In addition, the name of the institution and the forwarding address must be included in the request. The name of the person making the request must also be included in the letter.  In addition, the person making the request will have to be properly identified through either a driver’s license or any sign picture ID.

29. Can someone else pick up my transcript for me?
