 Education Unit Fingerprinting Policies & Procedures

 
All candidates must be fingerprinted upon acceptance into one of Queens College’s education programs. Candidates need to be fingerprinted prior to the first day of a course that requires fieldwork. Candidates will not be allowed to begin any fieldwork (i.e., fieldwork observations, initial clinical experience, student teaching, internship, or practicum) without being fingerprinted.
 
Please follow the instructions for the section that pertains to your situation.

Please note, the instructions below have been updated to reflect changes in process due to COVID-19:


I. Students who require NYC DOE clearance (NYC DOE Schools)
 

Self-register with the NYCDOE PETS Office  

a)    Send an e-mail to the Personnel Eligibility Tracking System (PETS) at PETS@schools.nyc.gov. Indicate that you are a Queens College student needing fingerprinting clearance for clinical work. Include your name, phone number, complete home address, date of birth, and email address.


b)    When you receive a reply from the NYCDOE PETS fingerprint office, follow all instructions provided in their email.

  • You will be asked to provide a letter stating you are a current Queens College student. This letter can be obtained via the QC Registrar’s Office.

Applicant Gateway 

 

c)    You will receive an e-mail from PETSAdminSupport@schools.nyc.gov. Read all information provided. Within this email is information on completing your online forms in the Applicant Gateway website.

 

d)    Follow all instructions on completing your Applicant Gateway forms. Remember to print the Fingerprint Referral Form.

 

Fingerprinting:

65 Court Street and the HR Connect Service Center remain closed.  Fingerprinting services will be available by appointment ONLY. Visitors without a verified appointment will be turned away.

 

e)    Email the HR Connect Service Center to request an appointment: HRCServiceCenter@schools.nyc.gov. The student will be added to the queue of people pending requests. The NYCDOE cannot provide estimates when a student will be allowed to schedule an appointment.

 

OSPRA 104

 

f)     After you have been fingerprinted, you will need to release your NYC DOE fingerprints to the New York State Education Department (for certification purposes). You must file an OSPRA 104 form (pdf). Fill out all pertinent information and mail to the indicated address at the bottom of the form.


 II. Students who require NY State Education Department fingerprinting 

(Long Island or NY State schools) 


 III. Students who have been previously fingerprinted by the NYC DOE

(Students already fingerprinted by NYC DOE and need to be added to

Queens College PETS database) 

Self-register with the NYCDOE PETS Office  

a)    Send an e-mail to the Personnel Eligibility Tracking System (PETS) at PETS@schools.nyc.gov. Indicate that you are a Queens College student who has been previously fingerprinted and request to have your information added to the Queens College database. Include your name, phone number, complete home address, date of birth, and email address.

 

b)    When you receive a reply from the NYCDOE PETS fingerprint office, follow all instructions provided in their email.

  • You will be asked to provide a letter stating you are a current Queens College student. This letter can be obtained via the QC Registrar’s Office.

c)    PETS will inform you when you have been added to the Queens College database.


 IV. Students fingerprinted by NY State Education Department who require their fingerprints be transferred to NYC DOE

Self-register with the NYCDOE PETS Office  

a)    Send an e-mail to the Personnel Eligibility Tracking System (PETS) at PETS@schools.nyc.gov. Indicate you are a Queens College student needing fingerprinting clearance for clinical work. Include your name, phone number, complete home address, date of birth, and email address.


b)    When you receive a reply from the NYCDOE PETS fingerprint office, follow all instructions provided in their email.

  • You will be asked to provide a letter stating you are a current Queens College student. This letter can be obtained via the QC Registrar’s Office.
Applicant Gateway

OSPRA 103

Until November 1, 2020, the following instructions are in effect:

   

a)    Click on the following link to download OSPRA 103 form: http://www.nysed.gov/common/nysed/files/ospra103.pdf.

 

b)    Complete sections I and II on OSPRA 103 form

 

c)    Scan (create a photo), and email signed form to the NYCDOE at: HRConnect@schools.nyc.gov. Typically, this request must be made in person at 65 Court St., but the NYCDOE is temporarily allowing the request to be submitted via email.

 

d)    NYCDOE will countersign section 3, but will not send a copy back.  Students should allow up to two weeks for the OSPRA request to be processed.


 V. Clearance Already Processed by Agency Other Than NYC DOE or NY State Education Department
 

If you had your clearance processed already by any agency other than the NYCDOE or New York State Department of Education (e.g., the New York City Department of Investigation (DOI) or Department of Health) and: 

a)    You will need NYC DOE security clearance - Refer to Section I above 

b)    You will need NY State Education Department Clearance - Refer to section II above 


 VI. International Students (F1 visa holder)
