How to obtain your QCard (the official ID of Queens College)

To obtain a QCard, you must have a CAMS (College Account Management System) account. If you do not have one, visit https://cams.qc.cuny.edu.

You can submit a Photo online for your QCard. Click here to submit your photo online.

Photos must be submitted online and approved by ITS. Once your photo has been approved, your QCard will be printed out and dropped off at the Public Safety Office located at the Main Gate of Campus for pick up. You must bring photo IDs as proof of identification (e.g., a driver’s license, a state-issued non-driver ID, or a passport) when picking up your QCard.

If you lose your QCard, there is a $10 replacement fee. Click here for instructions regarding making an online payment. Online payment only accepts MasterCard and Visa.

To ensure the durability and validity of your QCard:

  • Keep it in a wallet or cardholder
  • Keep it away from magnets and computers to avoid magnetic damage
  • Do not put it cell phone case of a phone capable of wireless charging as wireless charging may demagnetize the card
  • Do not punch holes in your card
  • Do not tamper with the information or picture printed on the card
  • Do not wash the card
  • Do not use the card for any purpose for which it was not designed

Once on campus, you will need to show your wellness check-approval confirmation email to gain access to campus and the QCard office. Public Safety staff will monitor the process and maintain social distancing protocols.

Multi-Factor Authentication

How to Setup Multi-Factor Authentication Using a Phone 

  • Sign into your QC email account with your password like you usually do. After you choose Sign in, you will be prompted for more information.
  • Choose Next.
  • From the Additional security verification page, select the following options:
  • Step 1: How should we contact you?
  • Select: Office phone
  • Select: United States as the country name if not already selected.
  • Enter the 10-digit phone number by which to be contacted. Do not include spaces, parentheses, or any other punctuation marks. Note: Please do not use your office number/extension.
  • Choose Next.
  • You will receive an automated call. Answer the call, and when instructed, press # on the phone keypad.
  • After your call is completed, you should see the following screen. Click Done to continue.

You will be directed to your QC email account. Your QC email account is now secured using Multi-Factor Authentication via office phone verification.

Student Guide to Vaccination Verification
  1. Log into your CUNYfirst account.
  2. Click on the Vaccination Verification tile on your CUNYfirst Homepage.
  3. If you have previously submitted verification information, you will see a list of your previous records and their status. To enter new information, such as a booster, click the Add New Record button. If you have not previously submitted any information, you will be taken straight to the page with the options.
  4. You will see three options in the Vaccine Information Options section – vaccinated (used for vaccination and booster details), medical exemption, and religious exception.


You are required to provide details on all vaccine doses you have received within the Vaccination Information Options section.

  1. Click the Vaccinated radio option in the Vaccine Information Options section.
  2. Select your Vaccination Brand from the dropdown menu.
  3. Depending on the vaccine brand you selected, you will see Vaccination Information fields for one or two doses generated below. Fill out the following details for the number of doses on the screen:
    • Input the date the dose was received in Vaccination Date field.
    • Select Vaccination Country and Vaccination Location from the dropdown menus.
    • If you selected USA as your Vaccination Country, include the State where you were vaccinated.
  4. If you have received a booster dose, click the Add Booster button.
  5. Fill out the information for your booster dose.
  6. If you received your vaccination in New York State and have a valid NYS Excelsior pass, click the checkmark in the NYS Excelsior Pass Attestation section and fill out your information details exactly as you provided them to the Excelsior App (FOR NEW YORK STATE EXCELSIOR PASS HOLDERS ONLY).
    Please note, this information will not be used to update any information in CUNYfirst.
  7. Upload your Vaccination Record in the Supporting Documents section. If your vaccine and booster information are on separate cards, ensure that both documents are uploaded. Select document type from the dropdown menu and click the Upload button to upload a document from your computer.
  8. If you have clicked the checkmark in the NYS Excelsior Pass Attestation, then click the Add a Row button to upload your NYS Excelsior Pass.
    Once your documents are uploaded, you can click the View link to review your uploaded files before submission.
  9. Review the Attestation section and check that you have read and agree to the University vaccination policy.
    If you are under 18 years old, the Attestation section must be completed by a parent or legal guardian.
  10. Click the Submit button to submit your information.
  11. Click OK in the confirmation pop-up window.
  12. Once submitted, you will see a pop-up message that your information has been successfully submitted. You will also see the Approval and Excelsior status fields appear at the top of the page. You may return to this page to view your approval status at any time.

Booster Only

  1. If your vaccination has already been approved and you would like to add your booster information, click the Add New Record button and select Vaccinated.
  2. Fill out the booster dose details in the generated fields.
  3. Upload the image of your booster dose card in the Supporting Documents section. Select document type from the dropdown menu and click the Upload button to upload the document from your computer.
  4. Review the Attestation section and check that you have read and agreed to the University vaccination policy. If you are under 18 years old, the Attestation must be completed by a parent or legal guardian.
  5. Click the Submit button.

Medical Exemption

Note: Medical exemption requests must be accompanied by a CUNY Medical Exemption Form completed by a medical professional.

  1. In the Vaccine Information Options section, select Medical Exemption.
  2. Provide your medical exemption reason in the Medical Reason Comment text box.
  3. Upload your supporting medical exemption documentation (including the CUNY Medical Exemption Form) in the Supporting Documents section by clicking the Upload link and selecting a file from your computer.
  4. Reason the Attestation section and check that you certify that all details and documentation you have provided are accurate and valid. If you are under 18 years old, the Attestation section must be completed by a parent or legal guardian.
  5. Click the Submit button to submit your information.
