In order to access the forms on this page, you will need to use your CUNY Login Credentials: @login.cuny.edu.
ISS Virtual Office Assistant DO NOT USE YOUR QC EMAIL (@qmail)
The ISS forms can only be accessed when signing in with CUNYfirst credentials.
Format: Firstname.Lastname (the last two digits of your CUNY ID#) @login.cuny.edu
- copy the Form link
- open an incognito window
- paste the link in the address bar of the new Incognito window
- this will then prompt the student to sign in
- now sign in using their CUNYfirst username format John.Doe13@login.cuny.edu
